Visit www.formfactory.sk

In the main menu, select the item “my account“. Enter your e-mail and password.

LOGIN DATA

To log in, use the e-mail you used to register with us for the first time.

In case of an invalid e-mail, ask the reception staff to change the login data.

If you have forgotten your password, you can reset it by clicking on “forgot your password?”

If you are a Multisport card user, create your account yourself or ask for login information at the reception.

HOW TO BOOK A CLASS
THROUGH THE WEB

Reservation is only possible online.

Choose timetable the Form Factory Club you wish to visit.

Then click on the lesson you want to book and confirm with the “sign up” button. If you are not logged in, log in.

The lesson can be booked earliest 72 hours before it starts and at the latest 5 minutes before it starts.

You can cancel the lesson no later than 2 hours before it starts. After this limit the cancellation of the booked class is not possible even through the reception. You can cancel the reservation the same way as you created it, by clicking the “cancel reservation” button, or in the list of reservations above the schedule.

RULES AN CONDITIONS
OF GROUP CLASSES RESERVATIONS

You can secure your place in the lesson by booking online through your client account. Booking through the reception is not possible. It is also possible to cancel the lesson online at least two hours before it starts. If you do not cancel the reservation within this limit and do not come to the lesson, or if you are late, your account will be blocked for the purpose of booking lessons for 5 days. In case of late arrival to the lesson, the lecturer has the right to exclude you from the lesson.

You can enter the lesson without a reservation after prior agreement with the instructor, provided that the capacity of the given lesson is not filled.

Please do not enter the room during the ongoing lesson.
